New Jersey stands out as being the first jurisdiction to remove capital punishment from its judicial system by means of legislative action, in 2006. A commission was appointed by New Jersey for evaluating its capital punishment system, and the commission by a vast majority recommended the removal of the capital punishment system. In Illinois, in 2000, consequent to a near miscarriage of justice, the Governor banned capital punishment, which still remains in place. Attempts to reinstate death penalty that was overturned in 2004 have been strongly resisted by the New York legislature (Dieter, 3). The scenario across America is almost the same, reflecting a clear indication that the death penalty is no longer popularly acceptable across the country. In twelve states death penalty finds no place in their statutes. Among the other 38 sta tes the application of death penalty is less than more, with probably the State of Texas remaining the sole staunch supporter of the continued use of capital punishment. With such clear evidence of mounting resistance to capital punishment across America, there can be no argument against the rising trend against the use of capital punishment in America (Revenge begins to seem less sweet). Book of the Courtier Essay â€Å"Whatever character an individual has determines how successful the community he belongs to will be† these lines are indeed true with what the â€Å"Book of the Courtier† suggests. Authored by an Italian soldier, and a statesman attached to the court of the duke of Milan and later in the service of Duke Urbino, Baldassare Castiglione, the entire book dwells in a central theme which includes having graceful behavior, especially that of the impression of effortlessness or referred to by the book as â€Å"sprezzatura. † This series of four books deals so much on how an individual should train himself in discipline so as to be able to reach the best person he could in a way that he could contribute so much to his own community. In the case of this book, Castiliogne puts it in the personification of a perfect courtier. On the third book though, the story evolves on how a court lady should carry herself and thus be able to serve her master in a way that they will never have any point of despising her service. Being clever is quite a clear attitude pointed in this part of the book. In addition to this, the wide differences between male and female genders is shown with emphasis. One part of these series of stories talks about the individuality of each men as I may quote: â€Å"I don’t want to be like the man who stripped down to his shirt and then jumped a shorter distance than he had in his greatcoat. So as far as I am concerned it is very fortunate that the hour is late, because as there is little time I shall have to say far less, and not having given the matter any thought, I will be excusable and allowed to say without being censured all the things that come to my mind. And now in order not to have to carry the burden any longer, let me start by saying that to recognize true perfection in anything is so difficult as to be scarcely possible; and this because of the opinions vary. Thus there are many who like to hear someone talking a great deal and who will call him an agreeable companion. Some will prefer reticence; others an active and restless man; others one who always acts with calmness and deliberation; and so everyone praises or condemns according to his own opinion, always Importance Placed on Individual or Society Page #2 camouflaging a vice under the name of a corresponding virtue; or a virtue under the name of a corresponding vice. † This lines shows how individual opinions affect how one person chooses to conduct himself in front of the other people in his community. Yet some choose to stick to what they believe in and dares to be different in some ways thus facing the judgment of his community men regarding his rather different being. Indeed, however a man chooses to deal with his own life defending his own beliefs as such, it will strongly influence his society’s views on things. A real â€Å"perfect courtier† is referred to as someone who can direct his own way. Someone who would not allow his principles be cracked by people who aims to put his â€Å"service to his master† into a lower state of excellence. A real noble man is the one who can decide for himself. From the smallest detail of how he should dress to the most complicated issue of how he should deal with his life. He that continuously struggles for his goals toward aspiring people. Someone who is able to measure his accomplishments with those of other people is the one considered to be truly successful. Everything must occur in such a reasonable way and with purpose. It is very important for each and every one of us to continue on pursuing our own goals to attain self excellence. As Sydney Bremer says in his book Successful Achievements: â€Å"In the assurance of strength, there is strength, and they are the weakest, however strong, who have no faith in themselves or their powers. Men often conquer difficulties because they think they can. Their confidence in themselves inspires confidence in others†. This is indeed true specially in connection with the theme of the book of Castiglione. It is thus true that what a person does as a separate part of his community is directly linked to other people living with him. Man, as the very basic part of a certain society determines both the norms and the idealism a community considers to be true and acceptable. On the other hand, aiming for the best, as suggested by the ideas of Castiglione is concerned with how we embed ourselves with the positive traits that lead to self excellence and also eliminating negative characters which may Importance Placed on Individual or Society Page #3 hinder us from doing what we are supposed to reach as our aims in life. One of which is â€Å"sprezzatura† which was translated in the modern English as â€Å"recklessness†. It came from an Italian root-word â€Å"sprezza† meaning contempt, disdain, or scorn. Having such attitudes wouldn’t make any young courtier a successful one at that. On the contrary, it will just lead him to failure and distress. His goals of giving his best service will surely be a waste if he continues to cultivate this character in his everyday dealings. This is so much true even to an individual who strives to be the best in what he does. As a whole, as we could closely see, the â€Å"Book of the Courtier† shows so much emphasis on the kind of living of an individual person. From the positive traits he must posses to the negative attitudes he must omit from his system so as to be able to meet the goal of self excellence. Women also were given emphasis to have certain independence from the authority of men in some ways. Giving them the free will, freedom of speech and freedom in deciding for what they want with their lives. Yes, as this book has been set at a long time past in England, this doesn’t necessarily mean that it doesn’t have any practical advise to the modern times we are presently living in today. Apparently, the contents of this book has a lot of advise enclosed for these generation which we are living in. As to what we could actually observe, many young people today lack the enthusiasm of actually establishing their own purpose in life. Sadly, only a few of the youth today endeavor to influence the community they are living in with positive contributions to the moral, spiritual, and even secular aspects of life in the society. Only a fewer percent of today’s population really strive for the aim of having self-excellence. Yes, sometimes, its not bad to be a bit perfectionist with ourselves. Knowing our limits of course, perfectionism usually lead us to self-excellence. As one saying implies : â€Å"Make perfection your aim and be satisfied with nothing less†. Yes, we must always remember to ask ourselves of what we ought to do as an individual, what we could improve on ourselves and thus be able to make a difference on the community we are living in. History:- Daniel Colladon first described this light fountain or light pipe in an 1842 article entitled On the reflections of a ray of light inside a parabolic liquid stream. This particular illustration comes from a later article by Colladon, in 1884. The principle that makes fiber optics possible, was first demonstrated by Daniel Colladon and Jacques Babinet in Paris in the early 1840s. John Tyndall wrote about the property of total internal reflection in an introductory book about the nature of light in 1870: When the light passes from air into water, the refracted ray is bent towards the perpendicular When the ray passes from water to air it is bent from the perpendicular If the angle which the ray in water encloses with the perpendicular to the surface be greater than 48 degrees, the ray will not quit the water at all: it will be totally reflected at the surface. The angle which marks the limit where total reflection begins is called the limiting angle of the medium. For water this angle is 48Â °27, for flint glass it is 38Â °41, while for diamond it is 23Â °42. The groundbreaking event happened in around 1965, Charles K. Kao and George A. Hockham of the British company Standard Telephones and Cables (STC) were the first to promote the idea that the attenuation in optical fibers could be reduced below 20 decibels per kilometer (dB/km), allowing fibers to be a practical medium for communication. They proposed that the attenuation in fibers available at the time was caused by impurities, which could be removed, rather than fundamental physical effects such as scattering. They correctly and systematically theorized the light-loss properties for optical fiber, and pointed out the right material to manufacture such fibers silica glass with high purity. This discovery led to Kao being awarded the Nobel Prize in Physics in 2009. NASA used fiber optics in the television cameras that were sent to the moon. At the time its use in the cameras was classified confidential and only those with the right security clearance or those accompanied by someone with the right security clearence were permitted to handle the cameras. In 1991, the emerging field of photonic crystals led to the development of photonic-crystal fiber which guides light by means of diffraction from a periodic structure, rather than total internal reflection. The first photonic crystal fibers became commercially available in 2000. Photonic crystal fibers can be designed to carry higher power than conventional fiber, and their wavelength dependent properties can be manipulated to improve their performance in certain applications. PROCESS OF MANUFACTURING OF FIBRE OPTICS:- Illustration of the modified chemical vapor deposition (inside) process Standard optical fibers are made by first constructing a large-diameter preform, with a carefully controlled refractive index profile, and then pulling the preform to form the long, thin optical fiber. The preform is commonly made by three chemical vapor deposition methods: inside vapor deposition, outside vapor deposition, and vapor axial deposition. With inside vapor deposition, the preform starts as a hollow glass tube approximately 40centimeters (16in) long, which is placed horizontally and rotated slowly on a lathe. Gases such as silicon tetrachloride (SiCl4) or germanium tetrachloride (GeCl4) are injected with oxygen in the end of the tube. The gases are then heated by means of an external hydrogen burner, bringing the temperature of the gas up to 1900K (1600Â °C, 3000Â °F), where the tetrachlorides react with oxygen to produce silica or germania (germanium dioxide) particles. When the reaction conditions are chosen to allow this reaction to occur in the gas phase throughout the tube volume, in contrast to earlier techniques where the reaction occurred only on the glass surface, this technique is called modified chemical vapor deposition. The oxide particles then agglomerate to form large particle chains, which subsequently deposit on the walls of the tube as soot. The deposition is due to the large difference in temperature between the gas core and the wall causing the gas to push the particles outwards (this is known as thermophoresis). The torch is then traversed up and down the length of the tube to deposit the material evenly. After the torch has reached the end of the tube, it is then brought back to the beginning of the tube and the deposited particles are then melted to form a solid layer. This process is repeated until a sufficient amount of material has been deposited. For each layer the composition can be modified by varying the gas composition, resulting in precise control of the finished fibers optical properties. In outside vapor deposition or vapor axial deposition, the glass is formed by flame hydrolysis, a reaction in which silicon tetrachloride and germanium tetrachloride are oxidized by reaction with water (H2O) in an oxyhydrogen flame. In outside vapor deposition the glass is deposited onto a solid rod, which is removed before further processing. In vapor axial deposition, a short seed rod is used, and a porous preform, whose length is not limited by the size of the source rod, is built up on its end. The porous preform is consolidated into a transparent, solid preform by heating to about 1800K (1500Â °C, 2800Â °F). The preform, however constructed, is then placed in a device known as a drawing tower, where the preform tip is heated and the optic fiber is pulled out as a string. By measuring the resultant fiber width, the tension on the fiber can be controlled to maintain the fiber thickness. Principle of operation:- An optical fiber is a cylindrical dielectric waveguide (nonconducting waveguide) that transmits light along its axis, by the process of total internal reflection. The fiber consists of a core surrounded by a cladding layer, both of which are made of dielectric materials. To confine the optical signal in the core, the refractive index of the core must be greater than that of the cladding. The boundary between the core and cladding may either be abrupt, in step-index fiber, or gradual, in graded-index fiber. Index of refraction: The index of refraction is a way of measuring the speed of light in a material. Light travels fastest in a vacuum, such as outer space. The actual speed of light in a vacuum is about 300,000 kilometres (186 thousand miles) per second. Index of refraction is calculated by dividing the speed of light in a vacuum by the speed of light in some other medium. The index of refraction of a vacuum is therefore 1, by definition. The typical value for the cladding of an optical fiber is 1.46. The core value is typically 1.48. The larger the index of refraction, the slower light travels in that medium. From this information, a good rule of thumb is that signal using optical fiber for communication will travel at around 200 million meters per second. Or to put it another way, to travel 1000 kilometers in fiber, the signal will take 5 milliseconds to propagate. Thus a phone call carried by fiber between Sydney and New York, a 12000 kilometer distance, means that there is an absolute minimum delay of 60 milliseconds (or around 1/16th of a second) between when one caller speaks to when the other hears. (Of course the fiber in this case will probably travel a longer route, and there will be additional delays due to communication equipment switching and the process of encoding and decoding the voice onto the fiber). Total internal reflection: When light traveling in a dense medium hits a boundary at a steep angle (larger than the critical angle for the boundary), the light will be completely reflected. This effect is used in optical fibers to confine light in the core. Light travels along the fiber bouncing back and forth off of the boundary. Because the light must strike the boundary with an angle greater than the critical angle, only light that enters the fiber within a certain range of angles can travel down the fiber without leaking out. This range of angles is called the acceptance cone of the fiber. The size of this acceptance cone is a function of the refractive index difference between the fibers core and cladding. In simpler terms, there is a maximum angle from the fiber axis at which light may enter the fiber so that it will propagate, or travel, in the core of the fiber. The sine of this maximum angle is the numerical aperture (NA) of the fiber. Fiber with a larger NA requires less precision to splice and work with than fiber with a smaller NA. Single-mode fiber has a small NA. TYPES OF FIBRE OPTICS:- Multi-mode fiber:- Fibers which support many propagation paths or transverse modes are called multi-mode fibers (MMF). Multi-mode fibers generally have a larger core diameter, and are used for short-distance communication links and for applications where high power must be transmitted. Fiber with large core diameter may be analyzed by geometrical optics. Such fiber is called multi-mode fiber. from the electromagnetic analysis . In a step-index multi-mode fiber, rays of light are guided along the fiber core by total internal reflection. Rays that meet the core-cladding boundary at a high angle , greater than the critical angle for this boundary, are completely reflected. The critical angle (minimum angle for total internal reflection) is determined by the difference in index of refraction between the core and cladding materials. Rays that meet the boundary at a low angle are refracted from the core into the cladding, and do not convey light and hence information along the fiber. The critical angle determines the acceptance angle of the fiber, often reported as a numerical aperture. A high numerical aperture allows light to propagate down the fiber in rays both close to the axis and at various angles, allowing efficient coupling of light into the fiber. However, this high numerical aperture increases the amount of dispersion as rays at different angles have different path lengths and therefore take different times to traverse the fiber. Single-mode fiber:- Those which can only support a single mode are called single-mode fibers (SMF). Single-mode fibers are used for most communication links longer than 550meters (1,800ft). The structure of a typical single-mode fiber. Core: 8Â µm diameter Cladding: 125Â µm dia. Buffer: 250Â µm dia. Jacket: 400Â µm dia. Fiber with a core diameter less than about ten times the wavelength of the propagating light cannot be modeled using geometric optics. Instead, it must be analyzed as an electromagnetic structure, by solution of Maxwells equations as reduced to the electromagnetic wave equation. The electromagnetic analysis may also be required to understand behaviors such as speckle that occur when coherent light propagates in multi-mode fiber. As an optical waveguide, the fiber supports one or more confined transverse modes by which light can propagate along the fiber. Fiber supporting only one mode is called single-mode or mono-mode fiber. The most common type of single-mode fiber has a core diameter of 8-10 micrometers and is designed for use in the near infrared. The mode structure depends on the wavelength of the light used, so that this fiber actually supports a small number of additional modes at visible wavelengths. Multi-mode fiber, by comparison, is manufactured with core diameters as small as 50 micrometers and as large as hundreds of micrometers. The normalized frequency V for this fiber should be less than the first zero of the Bessel function J0 (approximately 2.405). Special-purpose fiber: Some special-purpose optical fiber is constructed with a non-cylindrical core and/or cladding layer, usually with an elliptical or rectangular cross-section. These include polarization-maintaining fiber and fiber designed to suppress whispering gallery mode propagation. Photonic-crystal fiber is made with a regular pattern of index variation (often in the form of cylindrical holes that run along the length of the fiber). Such fiber uses diffraction effects instead of or in addition to total internal reflection, to confine light to the fibers core. The properties of the fiber can be tailored to a wide variety of applications. APPLICATIONS OF FIBRE OPTICS:- Optical fiber communication: Optical fiber can be used as a medium for telecommunication and networking because it is flexible and can be bundled as cables. It is especially advantageous for long-distance communications, because light propagates through the fiber with little attenuation compared to electrical cables. This allows long distances to be spanned with few repeaters. Additionally, the per-channel light signals propagating in the fiber have been modulated at rates as high as 111 gigabits per second by NTT, although 10 or 40Gb/s is typical in deployed systems. Each fiber can carry many independent channels, each using a different wavelength of light (wavelength-division multiplexing (WDM)). The net data rate (data rate without overhead bytes) per fiber is the per-channel data rate reduced by the FEC overhead, multiplied by the number of channels. For short distance applications, such as creating a network within an office building, fiber-optic cabling can be used to save space in cable ducts. This is because a single fiber can often carry much more data than many electrical cables, such as 4 pair Cat-5 Ethernet cabling. Fiber is also immune to electrical interference; there is no cross-talk between signals in different cables and no pickup of environmental noise. Non-armored fiber cables do not conduct electricity, which makes fiber a good solution for protecting communications equipment located in high voltage environments such as power generation facilities, or metal communication structures prone to lightning strikes. They can also be used in environments where explosive fumes are present, without danger of ignition. Wiretapping is more difficult compared to electrical connections, and there are concentric dual core fibers that are said to be tap-proof. Fiber optic sensors :- Fibers have many uses in remote sensing. In some applications, the sensor is itself an optical fiber. In other cases, fiber is used to connect a non-fiberoptic sensor to a measurement system. Depending on the application, fiber may be used because of its small size, or the fact that no electrical power is needed at the remote location, or because many sensors can be multiplexed along the length of a fiber by using different wavelengths of light for each sensor, or by sensing the time delay as light passes along the fiber through each sensor. Time delay can be determined using a device such as an optical time-domain reflectometer. Optical fibers can be used as sensors to measure strain, temperature, pressure and other quantities by modifying a fiber so that the quantity to be measured modulates the intensity, phase, polarization, wavelength or transit time of light in the fiber. Sensors that vary the intensity of light are the simplest, since only a simple source and detector are required. A particularly useful feature of such fiber optic sensors is that they can, if required, provide distributed sensing over distances of up to one meter. Extrinsic fiber optic sensors use an optical fiber cable, normally a multi-mode one, to transmit modulated light from either a non-fiber optical sensor, or an electronic sensor connected to an optical transmitter. A major benefit of extrinsic sensors is their ability to reach places which are otherwise inaccessible. An example is the measurement of temperature inside aircraft jet engines by using a fiber to transmit radiation into a radiation pyrometer located outside the engine. Extrinsic sensors can also be used in the same way to measure the internal temperature of electrical transformers, where the extreme electromagnetic fields present make other measurement techniques impossible. Extrinsic sensors are used to measure vibration, rotation, displacement, velocity, acceleration, torque, and twisting. Other uses of optical fibers:- Light reflected from optical fiber illuminates exhibited model Fibers are widely used in illumination applications. They are used as light guides in medical and other applications where bright light needs to be shone on a target without a clear line-of-sight path. In some buildings, optical fibers are used to route sunlight from the roof to other parts of the building . Optical fiber illumination is also used for decorative applications, including signs, art, and artificial Christmas trees. Swarovski boutiques use optical fibers to illuminate their crystal showcases from many different angles while only employing one light source. Optical fiber is an intrinsic part of the light-transmitting concrete building product, LiTraCon. Optical fiber is also used in imaging optics. A coherent bundle of fibers is used, sometimes along with lenses, for a long, thin imaging device called an endoscope, which is used to view objects through a small hole. Medical endoscopes are used for minimally invasive exploratory or surgical procedures (endoscopy). Industrial endoscopes used for inspecting anything hard to reach, such as jet engine interiors. In spectroscopy, optical fiber bundles are used to transmit light from a spectrometer to a substance which cannot be placed inside the spectrometer itself, in order to analyze its composition. A spectrometer analyzes substances by bouncing light off of and through them. By using fibers, a spectrometer can be used to study objects that are too large to fit inside, or gasses, or reactions which occur in pressure vessels. An optical fiber doped with certain rare earth elements such as erbium can be used as the gain medium of a laser or optical amplifier. Rare-earth doped optical fibers can be used to provide signal amplification by splicing a short section of doped fiber into a regular (undoped) optical fiber line. The doped fiber is optically pumped with a second laser wavelength that is coupled into the line in addition to the signal wave. Both wavelengths of light are transmitted through the doped fiber, which transfers energy from the second pump wavelength to the signal wave. The process that causes the amplification is stimulated emission. Optical fibers doped with a wavelength shifter are used to collect scintillation light in physics experiments Optical fiber can be used to supply a low level of power (around one watt) to electronics situated in a difficult electrical environment. Examples of this are electronics in high-powered antenna elements and measurement devices used in high voltage transmission equipment. USES:- Optical fibers are widely used in fiber-optic communications, which permits transmission over longer distances and at higher bandwidths (data rates) than other forms of communications. Fibers are used instead of metal wires because signals travel along them with less loss, and they are also immune to electromagnetic interference. Fibers are also used for illumination, and are wrapped in bundles so they can be used to carry images, thus allowing viewing in tight spaces. Specially designed fibers are used for a variety of other applications, including sensors and fiber lasers. Light is kept in the core of the optical fiber by total internal reflection. This causes the fiber to act as a waveguide. ADVANTAGES OF FIBRE OPTICS: We knowthe electrical signals travel pretty well in metal cables but nothing compares to light inoptical fibre. If we have to list the most outstanding advantages of using light as a carrier and optical fibres as transmission channels these may be some of them: Great bandwidth available to transmit information. You can easily use many GHz of bandwidth limitations being mostly related to electronics in the transmitters and the receivers. Low attenuation of the light travelling through optical fibres. Light can travel many kilometres in an optical fibre with little attenuation and without using amplifiers/repeaters or having them spaced a lot more than amplifiers in coaxial cables for example. Immunity to interferences. Optical fibres are made of glass not of any metal which makes them immune to any kind of electromagnetic interference. Galvanic isolation. Since they are not metallic they dont establish electrical contactbetween emitter and receiver nor create any capacitance along the length of the cable. Classroom Discipline and Management Philosophy Essay -- Classroom Manag

The goal in our classroom is to teach self-discipline, responsibility, cooperation, and problem-solving skills. Common causes of misbehavior include an attempt to meet unmet needs (belonging, significance, fun); a lack of needed skills (social and academic); inappropriate expectations; lack of structure; and a lack of sense of relevance. We will be focusing on non-punitive solutions to problems based on kindness, firmness, dignity and respect. True discipline comes from an internal locus of control (self-discipline), not an external locus of control (punishments and rewards inflicted by someone else). Some of the strategies used in our classroom will be proactive and some will be reactive. Many proactive strategies will be in place so that the use of reactive strategies is limited. These strategies include, but are not limited to: . The Aging Process in Katherine Mansfields Miss Brill Essay -- Miss Bri

Katherine Mansfield's "Miss Brill" perfectly captures the phases one's mind goes through when faced with becoming old. Elderly people tend to be nostalgic, even sentimental about their youth. In later years, the nostalgia can develop into senility or fantasy. The ermine fur in "Miss Brill" is the catalyst of her nostalgia and symbolizes the passing of time in three stages: an expectant youth, a vital adulthood, and finally, a development into old age and fantasy. The story opens with Miss Brill's excitement that the "season" has arrived for social engagements; perhaps it is the tourist season when the ladies debut their latest fashions. With all the expectancy of a young girl looking forward to courtship, Miss Brill unpacks her prized and most fashionable possession, the ermine fur. While unpacking the fur, the reader is aware that Miss Brill is lapsing into elderly nostalgia because she speaks to the fur in such delighted tones. Neurological System (headaches, head injuries, dizziness, convulsions, tremors, weakness, numbness, tingling, difficulty speaking, difficulty swallowing, etc., medications): Patient is alert, awake and oriented. Denies headaches, head injuries, dizziness, seizures, tremors, migraine, difficulty in speech and swallowing. No history of falls. Patient does mention that he has numbness and tingling of fingers and toes occasionally. Takes Gabapentin 100mg orally three times a day. Head and Neck (pain, headaches, head/neck injury, neck pain, lumps/swelling, surgeries on head/neck, medications): Patient denies neck or head injuries, denies swelling or lumps on neck and head, Denies neck pain or headaches. Eyes (eye pain, blurred vision, history of crossed eyes, redness/swelling in eyes, watering, tearing, injury/surgery to eye, glaucoma testing, vision test, glasses or contacts, medications): Patient wears eyeglasses that are with him. Bilateral cataract surgery done in June 2013. Regular v ision checks after surgery done in November 2013 and at present he is not on any medications at home. Ears (earache or other ear pain, history of ear infections, discharge from ears, history of surgery, difficulty hearing, environmental noise exposure, vertigo, medications): No complaints of ear pain, infection, surgery tinnitus due to noise, or vertigo noted. Patient is taking Aspirin 81mg orally daily, Lopressor 25mg orally daily, and Plav ix 75mg orally daily. Thorax and Lungs (cough, SOB, pain on inspiration or expiration, chest pain with breathing, history of lung disease, smoking history, living/working conditions that affect breathing, last TB skin test, flu shot, pneumococcal vaccine, chest x-ray, medications): Has occasional cough that could be due to change of climate. Denies shortness of breath or pain with breathing. Denies smoking and no history of lung disease is noted. Immunized for Influenza and Pneumonia on 10/14/2013. Patient was in ER in March for cough and fever and x-ray of the chest showed no abnormalities at that time. Musculoskeletal System (joint pain; stiffness; swelling, heat, redness in joints; limitation of movement; muscle pain or cramping; deformity of bone or joint; accidents or trauma to bones; back pain;  difficulty with activity of daily living, medications) Denies any symptoms of joint problems and does not take any medications at home. Patient is independent and requires no assistance for activities of daily living. His wife and he take walks on a daily basis for 20 minutes. Gastrointestinal System (change in appetite – increase or loss; difficulty swallowing; foods not tolerated; abdominal pain; nausea or vomiting; frequency of BM; history of GI disease, ulcers, medications) Denies any gastro-intestinal disease, ulcers, or diabetes. Consumes low sodium diet with no added salt three times a day and a bedtime snack. Includes plenty of vegetables and fruits in his diet. No swallowing problems noted. No complaints of nausea, vomiting or diarrhea noted. Patient has daily bowel movement and reports that it is brown in color. Denies use of stool softener or laxative. An Endoscopy and Colonoscopy was done in January 2014 and no abnormalities noted at that time. Genitourinary System (recent change, frequency, urgency, nocturia, dysuria, polyuria, oliguria, hesitancy or straining, urine color, narrowed stream, incontinence, history of urinary disease, pain in flank, groin, supra pubic region or low back) Denies pain or any urinary problems. Patient verbalizes increased frequency of urination due to Lasix. Patient wakes up twice at night to urinate but he is continent of bladder. Per patient no prostate problem noted. Last prostate exam was done in February 2014. Physical Examination (Comprehensive examination of each system. Record findings.) Neurological System (exam of all 12 cranial nerves, motor and sensory assessments): Patient is awake, alert, and oriented with no memory loss. Patient is calm, cooperative and pleasant. Judgment is intact. Patients speaks clearly and in full sentences. No difficulty noted while speaking. No swallowing problems noted. Patient has a steady gait with full strength. Sensations present in all extremities. Complaints of occasional numbness and tingling of fingers and toes but denies upon examination. Head and Neck (palpate the skull, inspect the neck, inspect the face, palpate the lymph nodes, palpate the trachea, palpate and auscultate the thyroid gland): Skull and neck are normal on examination. No deformities or hematoma noted. No lymph nodes identified on palpation. Adam’s apple present. Trachea is normal on palpation. Eyes (test visual acuity, visual fields, extra ocular muscle  function, inspect external eye struct ures, inspect anterior eyeball structures, inspect ocular fundus): Patient has eyeglasses with him. Patient is able to open and close his eyelids. Pupil is round and reaction to light is constriction to both eyes. Denies any blurring, watering, or tearing of the eyes. No redness or infection noted. Ears (inspect external structure, otocopic examination, inspect tympanic membrane, test hearing acuity): Hard of hearing right ear with no hearing aid. As per patient the physician had recommended hearing aid for the right ear but patient did not wish to use it. Otoscopic examination revealed normal ear canals and eardrums with minimal amount of earwax. Nose, Mouth, and Throat (Inspect and palpate the nose, palpate the sinus area, inspect the mouth, inspect the throat): Nose, mouth and throat are normal on examination. On palpation no pain noted to sinuses. The upper and lower dentures fit well on the patient and do not become loose while talking or chewing. Skin, Hair and Nails (inspect and palpate skin, temperature, moisture, lesions, inspect and palpate hair, distribution, texture, inspect and palpate nails, contour, color, teach self-examination techniques): No skin break down or rashes or lesions noted on inspection of the skin. Color is normal to ethnicity. Skin is warm, dry an d intact. Mucus membranes are pink and moist. Hair is gray and no alopecia noted. Texture of hair is soft to touch, no split ends noted. Kept short and clean. No ingrown nails or cracked nails noted. Nails are well groomed and pink in color. Patient verbalizes examining the skin and nails everyday while taking a shower. Breasts and Axilla (deferred for purpose of class assignment) Peripheral Vascular and Lymphatic System (inspect arms, symmetry, pulses; inspect legs, venous pattern, varicosities, pulses, color, swelling, lumps): Bilateral upper extremities are warm, symmetrical with bilateral radial pulses 2+. Bilateral lower extremities are warm, symmetrical without any discoloration. No varicose veins noted. Bilateral pedal pulses 2+. A trace of edema is noted on both ankles and feet. Cardiovascular System (inspect and palpate carotid arteries, jugular venous system, precordium heave or lift, apical impulse; auscultate rate and rhythm; identify S1 and S2, any extra heart sounds, murmur): Carotid arteries are normal with pulse 2+. No jugular vein distension noted. Apical pulse is 82 beats per minute, BP of 150/80 mm of Hg. Heart sounds S1 and S2 are on auscultation. No murmur or extra heart sound noted. EKG shows a  Normal Sinus Rhythm. Thorax and Lungs (inspect thoracic cage, symmetry, tactile fremitus, trachea; palpate symmetrical expansion;, percussion of anterior, lateral and posterior, abnormal breathing sounds): Thoracic cage is normal and symmetrical. No abnormality noted on palpation and percussion. Breath sounds are clear and equal on auscultation in all lung fields. Respirations are even, regular and unlabored. Patient has occasional nonproductive cough relieved by cough medicine. Respiratory rate is 18/minute and Oxygen saturation is 99% on room air. Pedal pulses is 2+. Nursing Diagnosis: Fluid Volume Excess manifested by edema of feet and ankles.
